Privacy & Confidentiality

This statement explains how your personal health information is handled. It is provided as required by the Ontario College of Pharmacists and Ontario privacy law (PHIPA). Please have it reviewed by the Designated Manager and legal counsel before launch.

Pharmly ("we," "us") is committed to protecting the privacy and confidentiality of your personal health information. Pharmly is delivered by a licensed, OCP-accredited Ontario pharmacy (identified in the footer), which acts as the health information custodian under Ontario's Personal Health Information Protection Act, 2004 (PHIPA).

What we collect

When you complete an assessment we collect the information you provide, which may include your name, date of birth, contact details, Ontario health card number, the condition you are asking about, your symptoms, medical history, allergies, and current medications. We also record the date and time of your request and your responses to the assessment questions.

Why we collect it

We use your information only to assess your condition, provide pharmacist care and, where clinically appropriate, prescribe and dispense treatment; to contact you about your assessment; to keep the records we are legally required to keep as a pharmacy; and to bill OHIP or process payment where applicable. We do not sell your information or use it for advertising.

Who sees it

Your information is accessed only by the licensed pharmacists and authorized pharmacy staff at Pharmly who are involved in your care, and only to the extent needed to provide that care. Prescriptions and assessment records are recorded in the pharmacy's regular records system, the same as an in-person visit.

How we protect it

Information submitted through this site is transmitted over an encrypted (HTTPS) connection. Sensitive fields, including your health card number and your assessment answers, are encrypted when stored. Access to records is limited to authorized staff, and our systems are maintained in line with applicable security requirements.

How long we keep it

We retain health records for the period required by Ontario pharmacy regulations and professional standards, after which they are securely destroyed.

Your rights

You have the right to access the personal health information we hold about you and to request a correction if you believe it is inaccurate. To make a request, or if you have a question or concern about your privacy, contact us using the details below. You also have the right to complain to the Information and Privacy Commissioner of Ontario (ipc.on.ca).

Pharmly handles minor ailments that fall within an Ontario pharmacist's scope of practice. It is not a substitute for emergency care. If you think you are having a medical emergency, call 911 or go to your nearest emergency department. A prescription is not guaranteed and is only provided when it is appropriate after the pharmacist's assessment. The assessment is free with a valid Ontario health card, or $30 without.
